The Greater Mercer Public Health Partnership (GMPHP) is a collaboration of hospitals, health departments, the Mercer County Department of Health and Human Services, and other not-for-profit organizations whose mission is to measurably improve the health of residents of the Greater Mercer County community.

The GMPHP recently completed the updated Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) for the greater Mercer County area. With the backing of the Community Advisory Board, these plans were unveiled in March 2016.

History

In 2012, the GMPHP conducted a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP) for all of Greater Mercer County. Four areas were identified as priorities:

1. Healthy Eating and Active Living

Our goal is to improve the health and well-being of the community by advocating for sustainable healthy lifestyle choices.

2. Chronic Disease

Our goal is to engage the community to help prevent and reduce chronic disease incidence and morbidity – for example, providing prevention information for residents with cancer, diabetes, heart disease, asthma.

3. Mental Health and Substance Abuse

Our goal is to improve access to quality mental health and substance abuse prevention, treatment and recovery services for all persons while reducing the associated stigma.

4. Transportation and Built Environment

Our goal is to increase the overall health and well-being of Greater Mercer County residents by enhancing safe, affordable, accessible options for people to move easily and freely within and between communities.

Governance

The GMPHP is governed by a core group of founding members, with the support of the Community Advisory Board, a large network of community leaders and organizations who function in partnership with the GMPHP.
